Neuro Rehab Services is committed to protecting your personal information.

All information is held with strictest confidentiality.

How do we store your information?

We store your personal information on a third party patient management system, Physiodiary, which is GDPR compliant and has cyber safety measures in place. We will obtain your personal information by phone, or through internal forms on Physiodiary, to minimise the amount of personal data being shared via email. Basic information may be obtained via email, but this will be kept to a minimal level. Information stored on Physiodiary will be archived once your treatment is complete.

Older records are paper based, and are securely stored as per GDPR guidelines. They will be archived once they have been stored for the legal time frame.

We use google workspace email domains, to keep your data as secure as possible. Emails will be deleted once no longer needed. Emails containing personal information being sent externally will be encrypted with a password.

Data obtained includes the following

  • Name

  • Address

  • Date of Birth

  • GP name and address

  • Contact information

  • Medical information

How do we use your information?

We will never share your information with third party companies, or use it for marketing.

We use your information in the following ways, and always with consent, unless in emergency situations (eg. calls to emergency services, or urgent discussions with your GP)

  • To set up your patient profile on Physiodiary

  • To allow full assessment, to ensure you are treated safely and effectively.

  • To send appointment reminders/ confirmation

  • To send in-built forms on Physiodiary to you.

  • To send reports to necessary professionals, with your awareness and consent; eg. solicitors, case managers, GP’s, consultants.

  • To discuss with other professionals involved, with your consent; eg. discussion of treatment plan with other therapists involved, to ensure a united approach with no conflicting approaches to treatment that may be detrimental to your rehabilitation.

  • To send invoices to you/ solicitors/ insurance companies/ case managers.

Changes to your personal details

Please inform us immediately if your contact details change, to allow us to update our system and ensure communication is secure and confidential.

Access to personal information

Under GDPR (2018) rules, you have a right to access personal information held by us. If you think information about you is inaccurate, we can update this, as we aim to have the highest standards of documentation.

  • To request personal information, you must submit your request in writing to Neuro Rehab Services, Room 4, Atherstone College, Ratcliffe Road, Atherstone, Warwickshire, CV9 1LF

  • This may incur an administration charge.

  • We are required to respond to you within 20 working days.

  • You will need to provide adequate information (for example full name, address, date of birth, and details of your request) to allow us to confirm your identity and respond appropriately to your request.

  • We are legally obliged to hold your data for 8 years (for adults) and until children reach the age of 21.

Information Commissioners Office

All Neuro Rehab Services staff members are registered with the Information Commissioners Office (ICO), which is a requirement of GDPR.

Our website contains links to other websites, to allow you to make informed choices about treatment of your condition. Our privacy policy only applies to our website, and not external websites.
